What can I expect from my case manager?
There are five key skills that you can expect from your case manager.
Positive relationship building
Case managers build trust and rapport with participants and
their caregivers. They develop person-centered relationships
based on mutual respect and understanding with people of all
backgrounds, lifestyle choices, and cultures.
Effective communication
Case managers are good communicators. They listen to the
people they work with and ensure all parties understand each
other.
Professionalism
Case managers are reliable, responsive, persistent, and
organized. They manage their time well.
Support and service facilitation
Case managers are knowledgeable about community supports
and disability services. They effectively assess needs, educate
participants and caregivers about options, help participants
access and maintain supports and services, and monitor
service quality.
Support team coordination
Case managers initiate and sustain teamwork with participants’
providers and other support team members. They ease
transitions from one support or service to another as
participants’ needs change.

into the five skill sets listed above. This list can help you reflect on what
working with a case manager should look and feel like.
