What is the purpose of this guide?
Living with a disability can mean having health care and support needs. You
might have to interact with a lot of doctors, nurses, and other support
people. You might be assigned a case management professional to help
you plan for and access the support you need.
This guide is for people with disabilities and their caregivers who interact
with case management professionals. It was created to help them:
• Better understand what they can expect of a case management
professional, and
• Give feedback on their case management professional’s performance.
The guide has four parts:
