Participant Feedback Survey
Purpose
The Participant Feedback Survey has a list of 19 things a case manager
should do. You can use this list to:
• Learn what to expect from a case manager
• Tell your case manager what you think they do well
• Tell your case manager what they can do better
Instructions
Think about the case manager you work with. For each statement, choose
the answer that best describes what you think about your case manager
during the last 3 months. You can ask for help from a caregiver to fill out
this survey. There are no right or wrong answers.
• Choose “Yes! Most of the time” if the statement describes what you
think about your case manager most of the time.
• Choose “Yes, some of the time” if the statement describes what you
think about your case manager sometimes, but not all the time.
• Choose “No, not really” if the statement does not describe what you
think about your case manager.
• Choose “I don’t know” if you don’t know how to answer the question,
or you don’t want to answer it.
