How do I know if my case manager is doing a good
job?
The next few pages contain two feedback surveys you can use to assess
your case manager.
Both surveys have 19 statements that align with the 19 case management
key skills. The survey can help you assess whether your case manager does
the things on the key skills list.
There are no right or wrong answers to these surveys. They are meant to
help you think about your experience with your case manager and provide
them with feedback, if you’d like.

If you are a person with a disability working with a case
manager, fill out the Participant Feedback Survey. If
you want, you can ask someone to help you fill out the
survey.

If you are the caregiver of a person working with a case
manager, fill out the Caregiver Feedback Survey to
provide feedback on a case manager you interact with
frequently.
Why should I fill out the survey?
The surveys can help you:
• Understand what to expect from a case manager
• Give your case manager feedback about what you think they do well and
what they can do better
What happens after I fill out the survey?
Once you complete the survey, you can decide what to do next. Pages 21 to
25 of this guide contain suggestions on how to share feedback with your
case manager or their supervisor.
