Who do case managers work with?
Case managers often work with:
• The person receiving services
• Caregivers
• The support team
What is a caregiver?
• A caregiver is a trusted family member, friend, or other
person who helps someone with a disability. Caregivers
may also work with the case manager to help set up
services and supports.
What is a support team?
• A support team is a group of workers who help with a
person’s health and daily needs. The team could
include doctors, therapists, social workers, job coaches,
and direct support workers.
