Who are case managers?
Case managers are people who work in health and
human services. Their job is to help you:
• Plan for services and supports
• Get the services you need
• Make informed decisions about your care
This guide uses the term case manager to describe these
workers. Case managers can have other job titles. Some
of those names are:
• Support coordinator
• Service coordinator
• Care manager
• Case worker
• Access specialist
Case managers may work for:
• Doctor’s offices
• Nonprofits or community organizations
• Health insurance companies
• Hospitals
• Government agencies, such as Medicaid or vocational
rehabilitation agencies
