Part 1: About this guide
What is this guide for?
Living with a disability can mean needing health care and
other supports. You may work with many people, like
doctors, nurses, and other support staff. You may also
have a case manager, who helps you plan for and get the
services you need.
This guide is for people with intellectual and
developmental disabilities (I/DD) and their caregivers who
work with case managers.
It explains what a case manager’s job is and what you can
expect of them.
When you know what to expect, you can:
• Better understand your case management services
• Speak up about what is working and what is not
• Take an active role in planning your care and supports
