Working with Case Managers
Part 1: Purpose, Background, and Terms
Who are case management professionals?
Case management professionals are health and human services workers
who coordinate other people’s health and wellness journeys. They help
people obtain services and make health-related decisions.
Throughout these materials, we will refer to these people as case
managers, although they can have different job titles. Common job titles
for case management professionals include:
• Case manager
• Care manager
• Service coordinator
• Support coordinator
• Rehabilitation counselor
• Case worker
• Access specialist
• Patient navigator
Case managers might work for a health care provider, nonprofit
organization, community-based organization, health insurance company,
hospital, or social services agency, such as Medicaid or a Vocational
Rehabilitation agency.
