Caregiver Feedback Survey
Purpose
This survey is designed for caregivers to
provide feedback on a case manager.
The survey lists 19 things a case manager
should do. You can use this survey to:
• Understand what to expect from a case
manager
• Give a case manager feedback about
things you think they do well and things
they could improve on
Instructions
Take a moment and think about the case manager you work with and your
experience with them over the past 3 months. Please choose the answer
that best matches your experience with your case manager.
For caregivers new to case management, this survey might help you learn
about the skills case managers should have. For more experienced
caregivers, the survey might help you assess whether the case manager is
doing everything they should be doing.
• Select “Yes! Most of the time” if the statement describes your
experience with your case manager most of the time.
• Select “Yes, some of the time” if the statement describes your
experience with your case manager sometimes, but not always.
• Select “No, not really” if the statement does not describe your
experience with your case manager.
• Select “I don’t know” if the statement does not apply to you, or you
don’t want to answer it.
There are no right or wrong answers. This survey should take less than 10
minutes to complete.
