How can I have a good conversation with my
case manager?
Here are some tips:
1. Set a time.
• Let your case manager know you would like to talk.
2. Be prepared.
• Think about what you want to say.
• You can use the Case Manager Discussion
Worksheet to help.
3. Share both positives and concerns.
• It does not feel good to hear only what is going wrong.
• Try to share what is going well, along with what you
would like to change.
