• You want something to change or improve
Talking with your case manager is often a good first step.
They may not know something needs to change unless
you tell them.
How can I share feedback in a helpful way?
You have options:
• Share the completed rating tool with your case
manager.
This will help you give detailed information. Your case
manager may ask you questions about your answers.
• Talk to your case manager.
You can talk about only the items that matter most to
you.
