How to use the list
This list helps you decide if the home, and those who run it, would be a good
choice for you to live. When you see the words 'I' or 'me' in the questions,
they refer to the person who will be receiving services.
Remember, this list is just a guide. It is not the only thing you should use to
choose a place to live.
Step 1. Get ready for your visit. Look at the questions in the list.
Think about which ones are most important to you. You don’t need
to ask all the questions—just the ones that matter most to you.
Step 2. When you visit, ask the staff the questions. You can also
look around the home to answer some questions.
Step 3. Look at your ratings.
If most of them say, “yes,” this could be a good place to live.
If most of them say, “maybe” or “no,” you might want to look
at other homes, if you can.
