• Will staff members from the program transport me to other
activities?
• Is there someone on staff members who can help me and my
family find resources (like benefits planning)?
• If the program cannot meet all my goals or interests, will someone
connect me with information about other community
organizations?
18 The program supports my personal relationships.
• How much time do I have to socialize with others, if I wish?
• Does the program support my relationships (such as romantic,
platonic, or familial), if I choose to have them? What does this
support look like?
19 The program uses communication methods that my family
and I prefer.
• What languages do staff members speak, and is communication
available in languages other than English, including sign language?
• What types of communication methods are available (phone calls,
emails, text messages, etc.)?
• Is internet service available to me? Are there rules about use?
• Can I use my own phone while in the program?
• Are families notified if there is an incident that affects me? How are
they notified? When are they notified?
20 The program updates me and my family on activities, my
progress, and other relevant topics.
• How often does the program give updates to family? What kinds of
updates do they provide? How are these updates communicated
(for example, through meetings, emails, or phone calls)?
• Does the program offer annual reviews of my performance, or can
I meet with staff members to discuss my performance when I want
to?
• When am I or my family made aware of staffing changes and
changes to the program?
• Do staff members consider my and my family’s suggestions and
input?
21 The program clearly explains financial expectations to me and
my family.
• How are the program services paid for?
• Does the program cost me anything?
• Will the program provide financial support to attend the program, if
needed (like for supplies)?
